Noncommunicable diseases

Focuses on cardiovascular disease, diabetes, cancer and chronic respiratory disease and their five shared main risk factors (tobacco use, harmful use of alcohol, unhealthy diet, physical inactivity and air pollution) as defined by the United Nations, this book provides a synopsis of one of the world’s biggest challenges of the 21st century. NCDs prematurely claim the lives of millions of people across the world every year, with untold suffering to hundreds of millions more, trapping many people in poverty and curtailing economic growth and sustainable development. While resources between and within countries largely differ, the key principles of surveillance, prevention and management apply to all countries, as does the need to focus resources on the most cost-effective and affordable interventions and the need for strong political will, sufficient resources, and sustained and broad partnerships. This compendium consists of 59 short and accessible chapters in six sections: (i) describing and measuring the burden and impact of NCDs / (ii) the burden, epidemiology and priority interventions for individual NCDs / (iii) social determinants and risk factors for NCDs and priority interventions / (iv) global policy / (v) cross-cutting issues / and (vi) stakeholder action.

Molecular Mechanisms of Parasite Invasion

All of the parasitic organisms highlighted in this new book represent medically important human pathogens that contribute significantly to the global burden of disease. As such there is intense interest in understanding the molecular basis of infection by these pathogens—not only with regard to their clinical relevance but also the fascinating biology they reveal. For most of the parasites discussed here the ability to penetrate biological barriers and/or to establish intracellular residence is critical to survival of the pathogen in the mammalian hosts. For other parasites, a tissue invasive phenotype is a key virulence determinant. In the ensuing 18 chapters, select members of this diverse set of protozoan parasites, as well as some examples of the extremely reduced fungal parasites classified as Microsporidia, are discussed within the context of the fascinating molecular strategies employed by these organisms to migrate across biological barriers and to establish residence within target host cells.

Irritable Bowel syndrome I.B.S

Irritable bowel syndrome (IBS) is a chronic gastrointestinal disorder with a range of symptoms that significantly affect quality of life for patients. Is currently one of the most common disorders of the digestive system in the Western society? Almost 2 out of 10 people suffer from IBS, and it is associated with abdominal pain, bloating and altered stool consistency and imposes a heavy burden for the affected patients. The difficulty of differential diagnosis and its treatment may significantly delay initiation of optimal therapy. Hence, persons with IBS often self-treat symptoms with non-prescribed pharmacological regimens and/or complementary and alternative medicines (CAM) and by modifying diet and daily activities.

Integrated Market and Credit Portfolio Models : Risk Measurement and Computational Aspects

Due to their business activities, banks are exposed to many different risk types. Aggregating various risk exposures to a comprehensive risk position is an important but up-to-date not satisfactorily solved task. This shortfall goes back to conceptual problems of constructing an appropriate risk model and to the computational burden of determining a loss distribution that comprises all relevant risk types. Peter Grundke deals with both problems. On the one hand, he extends a standard credit portfolio model by correlated interest rate and credit spread risk. The analysis shows that the economic capital needed as a buffer to absorb unexpected losses in a portfolio can be severely underestimated when relevant market risk factors are neglected. On the other hand, computational aspects are addressed.

Hepatitis C virus-host interactions and therapeutics : Current insights and future perspectives

The burden of hepatitis C virus (HCV) infection on the public health care system continues to remain significant despite the remarkable progress made in HCV therapeutics in the recent past. There are now almost a dozen oral interferon-free direct-acting antivirals available for the treatment of hepatitis C virus infection. Despite advances in the treatment of HCV, therapeutic gaps remain that are yet to be fully explored. Researchers and scientists still strive to understand virus-host interactions to map the disease’s progression along with extrahepatic manifestations and virus invasion strategies impacting the host’s immune system. Gastroesophagal reflux disease (GERD)

Gastroesophageal reflux disease (GERD) is a common disease with a high prevalence worldwide and a great socioeconomic burden. GERD is a multifactorial disease in which anatomical and functional factors both play a pathogenetic role. The disease can manifest in various symptoms which can be grouped into typical, atypical and extra-esophageal symptoms. The pathophysiology of GERD is complex and not completely understood. An abnormal LES pressure and increased reflux during transient LES relaxations are believed to be key etiologic factors. Prolonged exposure of the esophagus to acid is another. Heartburn and acid regurgitation are the most common symptoms of GERD, although pathologic reflux can result in a wide variety of clinical presentations.

From Assessing to Conserving Biodiversity : Conceptual and Practical Challenges

This book despite increasing communication, accelerating policy and management responses, and notwithstanding improving ecosystem assessment and endangered species knowledge, conserving biodiversity continues to be more a concern than an accomplished task. Why is it so? The overexploitation of natural resources by our species is a frequently recognised factor, while the short-term economic interests of governments and stakeholders typically clash with the burdens that implementing conservation actions imply. But this is not the whole story. This book develops a different perspective on the problem by exploring the conceptual challenges and practical defiance posed by conserving biodiversity, namely: on the one hand, the difficulties in defining what biodiversity is and characterizing that “thing” to which the word ‘biodiversity’ refers to; on the other hand, the reasons why assessing biodiversity and putting in place effective conservation actions is arduous.

Formulating poorly water soluble drugs

A pharmaceutical scientist’s approach toward solubility enhancement of a poorly water-soluble molecule typically includes detailed characterization of the compound’s physiochemical properties, solid-state modifications, advanced formulation design, non-conventional process technologies, advanced analytical characterization, and specialized product performance analysis techniques. The scientist must also be aware of the unique regulatory considerations pertaining to the non-conventional approaches often utilized for poorly water-soluble drugs. One faced with the challenge of developing a drug product from a poorly soluble compound must possess at a minimum a working knowledge of each of the above mentioned facets and detailed knowledge of most. In light of the magnitude of the growing solubility problem to drug development, this is a significant burden especially when considering that knowledge in most of these areas is relatively new and continues to develop.

Epilepsy has remained a significant social concern and financial burden globally. It is the most common neurological disease of the brain. Around 1% of the people worldwide have epilepsy and this disease affects people of all ages, Although the different types of epilepsy vary greatly, medication appropriate can control seizures in about 70% of patients, Medications are mainstays in controlling epileptic seizures, Partial seizures, which are the most common seizure type in adults, can be effectively controlled by virtually all the standard and newer antiepileptic drugs (AEDs). For the generalized epilepsies, valproate remains the drug of choice.

Enhanced Living Environments : Algorithms, Architectures, Platforms, and Systems

Ambient Assisted Living (AAL) is an area of research based on Information and Communication Technologies (ICT), medical research, and sociological research. AAL is based on the notion that technology and science can provide improvements in the quality of life for people in their homes, and that it can reduce the financial burden on the budgets of the healthcare providers. The concept of Enhanced Living Environments (ELE) refers to the AAL area that is more related with ICT. Effective ELE solutions require appropriate ICT algorithms, architectures, platforms, and systems, having in view the advance of science in this area and the development of new and innovative solutions. The aim of this book is to become a state-of-the-art reference, discussing progress made, as well as prompting future directions on theories, practices, standards, and strategies related to the ELE area.

Traditionally, aspect-oriented software development (AOSD) has focused on the implementation phase of the software lifecycle: aspects are identified and captured mainly in code. Therefore, most current AOSD approaches place the burden for aspect identification and management on the programmer working at low levels of abstraction. However, aspects are often present well before the implementation phase, such as in domain models, requirements and software architecture. Identification and capture of these early aspects ensure that aspects related to the problem domain (as opposed to merely the implementation) will be appropriately captured, reasoned about and available. This offers improved opportunities for early recognition and negotiation of trade-offs and allows forward and backward aspect traceability. This makes requirements, architecture, and implementation more seamless, and allows a more systematic application of aspects.

Dual Income Tax : A Proposal for Reforming Corporate and Personal Income Tax in Germany

In its Annual Report 2003/2004, the German Council of Economic Experts launched a dual income tax as an option for a fundamental tax reform in Germany. In February 2005, the German government appointed the Council to prepare a detailed report on economic effects of a business tax reform, with special emphasis on a dual income tax. With regard to the latter, conceptual problems of tax law and of tax administration were to be addressed as well as possible transitional problems when implementing a dual income tax. This book presents an English version of the original report completed in April 2006.

This study is about developing a secure, autoscaling military postponement e-service in Syria, that allows Syrian citizens to conveniently access services provided by the Syrian Recruitment Department conveniently through their smartphones. Currently, many Syrian citizens need to use the services offered by the Recruitment Department, resulting in overcrowding in a single location for similar purposes. This situation places a significant burden on both citizens and the government. The mobile application will facilitate various services such as enlistment and postponing military service by employing a well-designed software architecture that ensures scalability and efficient distribution of these services.
