EQ-5D value sets
For various reasons, cost benefit analysis is usually rejected in favour of cost-effectiveness or cost-utility analyses, often involving the estimation of the incremental cost per Quality Adjusted Life Year (QALY) gained (Drummond et al, 2005). The estimation of QALYs gained requires valuations for all relevant health states on a scale anchored at 1 = Full health and 0 = Dead. The EQ-5D is widely used in this context and a number of value sets are available for all the health states generated by the EQ-5D descriptive system. These can be readily applied to health outcomes measured as EQ-5D profiles. EQ-5D has become one of the valuation approaches recommended by several reimbursement authorities and academic bodies in European countries
Epiphyseal Growth Plate Fractures
This comprehensive reference work covers all aspects of growth plate fractures and their complications. Following general reviews of growth plate fractures, 21 chapters deal with each epiphyseal growth plate in the body. All of these chapters are constructed similarly for easy and quick retrieval of the required information. The main emphasis is on evaluation (diagnosis) and management (treatment). A separate section is devoted to premature partial physeal arrest, as this is by far the most common and feared complication of a growth plate fracture and its treatment is involved and controversial. The case studies included are often based on 20- to 30-year follow-ups, revealing cases that originally appeared to be quite satisfactory at the conclusion of growth but were found to have turned out quite poorly years later.
Environmental Security in Harbors and Coastal Areas : Management Using Comparative Risk Assessment and Multi-Criteria Decision Analysis
This book explores the challenges facing coastal areas during the next few decades and the difficult decisions needed to prevent a repeat of the past. Establishing, maintaining or enhancing a sense of environmental security in different coastal regions and improving the management of critical infrastructure will require (i) matching human demands with available environmental resources; (ii) recognition of environmental security threats and infrastructure vulnerabilities; and, (iii) identification of the range of available options for preventing and/or minimizing natural disasters, technological failures, and/or terror actions. This book presents the environmental history of the Delta of the lowland rivers Rhine and Meuse, an ecological story on evolving human–environmental relations coping with climate change and sea-level rise. It offers a combination of in-depth ecology and environmental history, dealing with exploitation of land and water, the use of everything nature provided, the development of fisheries and agriculture, changes in biodiversity of higher plants, fish, birds, mammals and invasive exotics. It is the first comprehensive book written in English on the integrated environmental history of the Delta, from prehistoric times up to the present day. It covers the l- acy of human intervention, the inescapable fate of reclaimed, nevertheless subs- ing and sinking polders, ‘bathtubs’ attacked by numerous floods, reclaimed in the Middle Ages and unwittingly exposed to the rising sea level and the increasing amplitude between high and low water in the rivers. The river channels, constricted and regulated between embankments, lost their flood plains, silted up, degraded and incised. Cultivation of raised bog deposits led to oxidation and compacting of peat and clay, resulting in progressive subsidence and flooding; arable land had to be changed into grassland and wetland. For millennia muscular strength and wind and water powers moulded the country into its basic form. From 1800 onwards, acceleration and scaling up by steam power and electricity, and exponential popu- tion growth, resulted in the erection of human structures ‘fixed forever’, and severe pressure on the environment.
Environmental Governance in Latin America
The multiple purposes of nature – livelihood for communities, revenues for states, commodities for companies, and biodiversity for conservationists – have turned environmental governance in Latin America into a highly contested arena. In such a resource-rich region, unequal power relations, conflicting priorities, and trade-offs among multiple goals have led to a myriad of contrasting initiatives that are reshaping social relations and rural territories. This edited collection addresses these tensions by unpacking environmental governance as a complex process of formulating and contesting values, procedures and practices shaping the access, control and use of natural resources. Contributors from various fields address the challenges, limitations, and possibilities for a more sustainable, equal, and fair development. In this book, environmental governance is seen as an overarching concept defining the dynamic and multi-layered repertoire of society-nature interactions, where images of nature and discourses on the use of natural resources are mediated by contextual processes at multiple scales.

Ennio De Giorgi : Selected Papers
The book contains a selection of 43 scientific papers by the great mathematician Ennio De Giorgi (1928-1996), which display the broad range of his achievements and his entire intellectual career as a problem solver and as a proponent of deep and ambitious mathematical theories. All papers are written in English and 17 of them appear also in their original Italian version in order to give an impression of De Giorgi’s original style. The editors also provide a short biography of Ennio De Giorgi and a detailed account of his scientific achievements, ranging from his seminal paper on the solution of Hilbert’s 19th problem to the theory of perimeter and minimal surfaces, the theory of G-convergence and the foundations of mathematics.

As urban areas keep growing, water infrastructure ages, and the requirements on environmental protection become more rigorous, there is a continual need for upgrading water pollution control facilities and restoring degraded urban waters. Such issues are addressed in this book by focusing on five major topics: (a) Upgrading stormwater management facilities, (b) Retrofitting / upgrading combined sewer overflow (CSO) facilities, (c) Optimising/upgrading sewage treatment plant performance, (d) Urban stream restoration, and (e) Challenges in restoring urban environment. Each chapter contains some overview papers followed by research or case study papers. Besides presentations of new approaches and accomplishments in the field of upgrading and restoration, several papers provide analysis of vast needs in this field in several countries of Central and Eastern Europe, which either recently joined the European Union (EU) or are preparing for accession, and need to comply with the existing EU directives dealing with environmental protection.
