Functional Hemodynamic Monitoring
Hemodynamic monitoring is one of the major diagnostic tools available in the acute care setting to diagnose cardiovascular insufficiency and monitor changes over time in response to interventions. However, the rationale and efficacy of hemodynamic monitoring to affect outcome has come into question. We now have increasing evidence that outcome from critical illness can be improved by focused resuscitation based on existing hemodynamic monitoring, whereas non-specific aggressive resuscitation impairs survival. Thus, this book frames hemodynamic monitoring into a functional perspective wherein hemodynamic variables and physiology interact to derive performance and physiological reserve estimates that themselves drive treatment. This philosophy, as well as the limitations and applications of common and evolving hemodynamic measures and their focused use in the care of critically ill patients are discussed, relevant to one underlying truth: No monitoring device, no matter how simple or sophisticated, will improve patient-centered outcomes useless coupled to a treatment which, itself, improves outcome.

Diabetic Kidney Disease
Presents the latest information on the clinical-pathological features of diabetic kidney disease. The data included is based on a cohort study of biopsy-proven diabetic nephropathy patients and nephrosclerosis patients, who were observed over a long term, and on the long-term registry for diabetic nephropathy (diabetic kidney disease) in Japan. It provides a clinical-pathological axis in clinical settings, including differential pathological/clinical diagnoses of CKD in diabetic patients (e.g. the presence of “classic” diabetic nephropathy and/or nephrosclerosis and/or other primary kidney diseases). The abundant biopsy specimens with long-term medical records provide a detailed pathological and clinical description. The book also includes urine-sample data for developing and validating possible candidates for novel biomarkers for diabetic kidney disease. Many countries, including Japan, have ageing populations, in which nephrosclerosis contributes to the progression of kidney lesions in patients with diabetic kidney disease. As such, a comparison of a diabetic nephropathy cohort with nephrosclerosis is indispensable to offer better treatments.

Breast cancer is a disease in which abnormal cells in the breast tissues multiply and form an invasive (or malignant) tumor. We will describe in our study several types of breast cancer, since they are classified into two main categories: “invasive” and “non-invasive,” or in situ. While invasive cancer has spread from the breast ducts or glands to other parts of the breast, non-invasive cancer has not spread from the original tissue. Our study will focus on the best tests that examine the breasts which are used to detect and diagnose breast cancer: Physical exam and health history, Clinical breast exam (CBE), Mammogram, Ultrasound exam, MRI (magnetic resonance imaging), Blood chemistry studies and Biopsy. We will include the value of such tests after breast cancer has been diagnosed, to find out if cancer cells have spread within the breast or to other parts of the body.
